OVERVIEW
Cameron Spine  is the official patient portal for Cameron Spine & Pain Management, a  licensed U.S. medical practice specializing in interventional spine and  pain care. It helps the practice's patients learn about spine  procedures, complete a pre-visit intake questionnaire, track their pain,  and reach the office. No account or login is required — all features  are open to explore.

MEDICAL APP AFFILIATION (Guideline 1.4.1 / 5.1.1)
This  app is published by and for a real, licensed medical practice. An  authorization letter on practice letterhead confirming the app's  affiliation with Cameron Spine & Pain Management is available on  request — reply via Resolution Center and we will provide it within one  business day.

HEALTH DATA USAGE
The optional Pain Intake  Questionnaire collects clinical information (pain characteristics,  neurological symptoms, functional-assessment scores, prior treatments,  imaging history) so the patient's care team can prepare for their visit.  This data is encrypted on the device (AES-256) and transmitted over TLS  to the practice's HIPAA-eligible systems. The app uses no third-party  advertising or tracking SDKs and does not sell data. Pain-tracker logs  and journal notes are stored locally on the device.

CONTACTS USAGE (why NSContactsUsageDescription is present)
Contacts  access is used for ONE optional feature only: "Invite Friends &  Family" in Settings. It is never accessed in the background or at  launch. The system permission prompt appears only after the user taps  Invite and chooses to pick someone from their contacts; the selected  name/number is used solely to send that single invitation. Declining the  permission has no effect on any other feature. To test: Settings tab →  "Invite Friends & Family."

EMERGENCY SCREENING BEHAVIOR (Guideline 1.4 Physical Harm)
The  intake questionnaire includes a red-flag screen. If answers suggest an  emergent condition (e.g., suspected cauda equina syndrome), the app  shows a hard-stop modal instructing the user to call 911 and seek  emergency care immediately, and notifies the clinical team. The app  states clearly that it is an educational/care-coordination tool and not a  substitute for emergency services.

The  policy covers HIPAA-aligned PHI handling, encryption in transit and at  rest, and a 7-year retention schedule. App Privacy answers in App Store  Connect match this policy.

ENCRYPTION / EXPORT COMPLIANCE
The  app uses only standard encryption (HTTPS/TLS and standard AES for data  protection), which qualifies for the export exemption  (ITSAppUsesNonExemptEncryption = false).
